JOB TITLE: LEAD HABILITATION TECHNITION II
|
SUMMARY :(Brief Statement of Purpose)
Under general direction and supervision of the Support Supervisor/Service Coordinator, performs para-professional work or routine difficulty in providing services to consumers based on individual service plans and providing personal care services to individuals with developmental disabilities. Performs related work as required.
DISTINGUISHING CHARACTERISTICS:
This classification is responsible for interacting with and providing a safe environment, including social and community services/activities, to service consumers. This classification is distinguished from all other direct care and service classifications by its responsibility to participate in daily living activities for the consumers who are in a residential care setting.
DUTIES/RESPONSIBILITIES: (Illustrative only – Work assignments may vary and will be communicated to the employee by the supervisor.)
- Coordinates, supervises and participates in the activities of a specialized service; provides support to staff and participates in daily activities essential to the completion of the service objectives.
- Provides a safe environment by communicating procedures with consumers, support coordinators, visitors and staff
- Observes consumers behavior’s and reports observations to professional staff
- Communicates and interacts with consumers on an individual and group basis
- Assists consumers with eating, showering, bathing, toileting, dressing, shaving, combing hair, shampooing, brushing teeth, cutting nails, and other daily living activities.
- Encourages consumers to participates in counseling, social, recreational, vocational and other activities identified in their treatment plan while promoting individual choice and decision making opportunities
- Reviews and implements approved policies, standards, services, and procedures
- Escorts and transports consumers to vocational, residential and social activities as identified, encourages use of public transportation systems when applicable, and runs consumer related errands
- Takes part in developing and implementing individualized service plans or person-centered plans for assigned consumers and ensures program directives are followed pursuant to State standards.
- Participants in staff and administrative meetings as well as in-service training
- Administers non-prescription and prescription medication (excluding IV medication) as prescribed by licensed medical professionals
- Maintains records, charts progress notes, records daily activities in communication logs, maintains records as needed and may be required to write reports.
- Reports unusual incidents on proper forms and informs appropriate staff regarding incidents involving vocational, day, social, recreational, behavioral, personal living and community services problems within required time-frames
- Provides direct services in crisis or emergency situations
- Observes and reports changes in consumer’s mental and physical health to enable appropriate intervention/prevention of problems.
- Operates equipment to aid in the assistance/movement of incapacitated consumers
- Acts as advocate in ensuring personal choice, freedoms, responsibility and support for consumers and aids in resolving problems as well as secures needed services for consumers.
- Performs housekeeping tasks such as cleaning, cooking, laundering, making beds, yard work, and other household chores or tasks required to support/service the consumer.
- Assists in performing various activities, including physical management procedures
- Organizes resources in ways that are life enhancing and meaningful for consumers
- Develops and maintains working relationships with all team members, including Service Coordinators, family members guardians, fiduciaries, advocates, etc;
- PERFORMS OTHER RELATED DUTIES AND ASSIGNMENTS AS REQUIRED
KNOWLEDGE AND SKILLS:
- Human behavior indicative of mental or physical disabilities
- Safety precautions used in transportation and home health care; licensing requirements in the State of Arizona
- Light housekeeping, basic home health functions, nutrition, and personal hygiene techniques
- Behavior modification techniques/behavioral treatment plans and the responsibility of an advocate
- Principles and practices of para-professional work with developmental disabilities and mental illness
- Establishing and maintaining effective working relationships
- Interacting with developmentally disabled consumers
- Principles and practices of self-determination and individual supports.
- Record keeping, writing reports and taking notes
- Administration of medication and awareness of possible side effects.
MINIMUM QUALIFICATIONS:
- Must be at least 18 years of age (licensing requirement). If driving is required for the position, must be 21 years old with no more than two (2) minor moving violations or one (1) accident within the past five (5) years.
- Possess a valid Arizona driver’s license and be insurable under the agency’s automobile policy.
- Minimum Qualifications: High school diploma or GED plus six (6) months experience in human services/behavioral health field. (Any combination of training and experience that demonstrated the necessary skills may be substituted for the aforementioned experience)
- Preferred Qualifications: Associate Arts degree in behavioral health from an accredited college; or and Associate’s degree in any field plus one (1) year experience in behavioral health.
- Have access to reliable, safe transportation with acceptable vehicle insurance to be used for agency travel as indicated by program if necessary.
- Be available by home phone, cell phone or pager for easy access by agency personnel
- Able to work flexible schedules: weekends, nights, days, evenings, partial shifts, 8/12/16 hour shifts.
- CPR and Standard First Aid
- Able to operate common office equipment and have basic computer skills, including word processing.
MENTAL AND EMOTIONAL REQUIREMENTS:
- Ability to effectively communicate in English both writing and verbally
- Able to positively interact and develop rapport with consumers and their families, professional support staff, and various levels of staff from community agencies.
- Able to maintain a calm, non-defensive, supportive attitude crisis or potential crisis situations.
PHYSICAL REQUIREMENTS:
- Must be fully ambulatory and able to lift consumers 9be able to lift minimum of 50 pounds).
- Must be able to climb stairs and assist consumers in moving household items if necessary.
- Must be able to assist consumers with household chores (involving many physical activities, including but not limited to kneeling, reaching, stretching, bending, etc., and the use of household cleaners).
- Must be able to endure outdoor temperatures and sudden temperature changes.
- Must be able to demonstrate competency in the following areas: Operate an agency transport vehicle; manual dexterity to keep documentation records; able to see and hear consumers in order to monitor consumer activities; perform crisis intervention techniques to prevent behavior harmful to the consumers or others which may require significant physical activity; assess and provide behavior management in crisis situations and call for assistance if needed; and be able to physically perform First Aid/CPR.
OTHER REQUIREMENTS:
Licenses and certificates: If a position requires driving a Palm View vehicle or rental car, a valid Arizona driver’s license (class D) is required with Palm View insurance clearance eligibility. CPR and First Aid certification from an accredited certifying body is required upon employment.
Special Notice Items: May be required to carry a pager/cell phone. In-service training sessions are required. Due to Department of Economic Security/division of Developmental Disabilities requirements, positions in this category are required to provide fingerprint clearance. Due to Department of health Services/Division of Behavioral Health requirements, some positions in this category are required to provide medical clearance. Department of Labor regulation CFR part 1910.1030 requires notification that this category may have a risk of exposure to blood-borne pathogens. Hepatitis B vaccine will be provided.
